The Mushroom is pretty much exactly as the name implies. It's basically a mushroom that walks on four legs and has a rather scary-looking face on it.
This creature will also only attack with its mushroom top and the two front legs. The front legs are used in a special attack, called "Running Kick." It involves a series of weak attacks towards the player. The Mushroom is no stronger or weaker than any of the other monsters, so there is really nothing to fear.
